During the process of qualifying for weight loss surgery, one of the very first
things you will need to do is call your insurance company and ask if your specific
policy excludes weight loss surgery. If your policy covers weight loss surgery,
then you will obtain a set of prior authorization requirements. These are
criteria which will need to be addressed before having final authorization for
surgery.
