SIGNALMENT: 12 year old F/S Labrador Retriever

HISTORY: Ladybird was spayed when she was very young. Since about 3 years of age, she has been treated for urethral sphincter mechanism incompetence to effect with phenylpropanolamine. The current complaint is sticky, odorless vulvar discharge. The owner sees the discharge most commonly after Ladybird urinates. Ladybird has been licking at her vulva more than usual for the last several weeks. She also is licking at her paws and scratching at her ears.

PHYSICAL:      T 101.4°F      P 100     R 36

EYES No abnormalities noted
EARS Both ear canals erythematous, contain foul-smelling brown discharge
ORAL CAVITY Grade III periodontal disease with gingival hyperplasia over upper canines
SKIN AND HAIRCOAT Localized alopecia on paws and in groin, erythema between toes on all four feet
LYMPH NODES No abnormalities noted
HEART / LUNGS Grade 4 / 6 cardiac murmur, holosystolic, louder on left side, lung sounds slightly increased in all fields
ABDOMINAL PALPATION No abnormalities noted - difficult to palpate due to size
GENITOURINARY Recessed vulva, mild perivulvar dermatitis, mucoid discharge caught in perivulvar hair
BODY CONDITION SCORE 8 / 9

To complete the exercise, create a written report, providing:
1) a synopsis of the case, as you might present to another clinician as you handed this case over to them - this is excellent practice for referral cases you will send to other hospitals in the future, and for communications with other veterinarians in facilities where you may work
2) a list or problems and rule-outs - this permits me to see your thought process
3) a list of diagnostic tests you would run and interpretation of results- diagnostic test results can be accessed through the links present on each case's web page. You may choose not to run all the tests available. Make sure you could defend all of your choices to the owner.
4) a treatment plan and estimate of costs for the owner.
